PLMS Student Athlete

All PLMS student-athletes must submit all required forms before competition.

These include:

• Sports Physical (for new athletes, e.g., 7th graders who have never played a sport at PLMS) OR Sports Physical Update Form only (for returning athletes, e.g., 8th graders with a sports physical form already on file)

• Proof of Insurance

• Concussion Form

• PLMS Athletic Policy

• Ensure all forms are submitted promptly to ensure eligibility for upcoming competitions. Thank you for your cooperation!
