Athletics

All PLMS student-athletes must submit the required forms before competition.

These include:

• A Sports Physical is required for ALL athletes (Any athlete, e.g., 6th - 8th grader who has never played an extracurricular sport at PLMS).

• Returning PLMS Athletes: A physical update form is required (for returning athletes, e.g., 6th - 8th grader with a sports physical form already on file).

Additional forms are required on an annual basis for every athlete:

• Proof of Insurance Form

• Concussion Form

• PLMS Athletic Policy

Ensure all forms are submitted promptly to ensure eligibility for upcoming competitions.

PLMS Sports Passes are available for $50. The pass covers all home middle school athletic events (excluding District Tournaments) and is valid for immediate family members within the household only.

Purchase in person (cash/check to PLMS Athletics) at Wednesday’s Football game or any subsequent middle school games. Alternatively, buy online via SchoolBucks, and cards will be distributed to families within 7 days of purchase.

(District employees and their immediate family get free entry to all middle school athletic events, excluding District Games.)
